*Note: At the local preliminary level, you do not need to complete Attachment
B - pages 17 and 18 of the State Contract dealing with Medical Information.
Adobe Reader is needed to complete these forms. Please note, you cannot
save this, any changes must be made at the time of completing each form. After
completing the forms, print them, make copies, and use them to enter the state
preliminaries.
Please send six copies of the Platform Essay and Resume to the individual preliminaries you enter.
Right click on any of the forms

of the form to your hard drive and complete at your convenience.
